Competed in my first 3 lift comp about a month or so ago, just at my usual gym.

Was warming up for the bench, for my final warm up loaded the bar up as per usual, unracked it and went to do the bench press only to fail with no spotter. Sat it on my chest for a second or two until some other competitors came and lifted it back onto the rack.

Finally realised it was 110kg NOT 90kg, felt like a bit of a goose but it happens...

Epic fail on my behalf.
 
Training for the city to surf half marathon 2013, lost 7kg (probably most of it muscle) and hurt my knee in the penultimate week so I couldn't participate at the event anyways. Was pretty mad.
 
Have done that once, seen it happen a few other times. Now I'm extra cautious and take no more than 20kg off at a time lol

Saw a young lass just about face plant doing box jumps on the bench after her squats today...

landed on her knees on the bench though before re-composing herself.

I tell you... her butt under that lycra... day-um!

Was mesmerising while resting in between sets. Hope I didn't come across as too obvious staring at her in the mirror:p

Pretty sure she snapped me, but just returned my gaze with one of her own... hnnngg...

this happened to me when i first hit the gym , here i was shy and intimated by all the huge blokes in the gym , for awhile i stayed to the edge of the gym not daring to enter the freeweights area

one day i got the courage to stroll over to the bench press , so i started unloaded one side , after all i had watched others do it so thought how hard could it be
next thing bang crash thud FFFUUK , the bar shot up like a catapult , everyone in the gym stopped and stared, management came rushing from all directions
 
and then just before christmas , me and a few blokes down the gym decided to do the 7 week challenge , a fitness challenge held by the gym involving bodyweight and kettlebell work outs , and running of course

did the first night not a problem , me and two others decided to meet up the next day to do some sand sprints , i was about 2minutes into it when i twisted my ankle so that ended the sand sprints for the day ,

thinking it was just a sprained ankle i did 2-weeks more of the 7 week challenge until i found out i had a broken bone in my ankle which explained why the sprained ankle wasn't getting better

so i ended up in plaster , the doctor didn't tell me i needed crutches to stay off my foot , so i went back to the gym and did some barbell curls and shattered the plaster, doctor wasn't too happy that is when he informed me i needed to stay off it and use crutches , so i was in plaster for a 2 more weeks until the specialist put me in a moon boot
 
Fell flat to my knees during a squat with 70kgs on my back

Fell backwards squatting my spotter failed also (Miscommunication) @trentzor

Got stuck under a bar loaded with 40kgs during hip thrusts, failed the lift then couldn't roll the bar off, was pinned.

Kicked a crossbar during crossfit and fractured my left tibia

Knocked the barbell off the hooks with a small amount of weight loaded and sledge hammered my ankle

Knocked the barbell off the hooks with the front of my head after doing a bodyweight squat warm up

Flicked a band off my foot during a kipping pull up and it flicked me somewhere that hurt....
 
got stuck under the 45 leg press , got distracted and lost my focus, it came crashing down , didn't have enough to throw the stoppers in place , had to crawl out , lucky the gym was empty

Don't be embarrassed, personally I think not being afraid to fail is such a good trait. When most people will rack their bench, I'll try one more even if I'm not confident I can re rack it, just because it feels like you push so much harder when you know you might drop it and be pinned under until someone helps you.

it was when i was new the gym , it was bad enough that i was lacking confidence and was intimated by others in the gym , the last thing i wanted was to draw attention to myself
 
I shat myself on a vertical leg press machine with diarrhea running all over me. Still finished the set.

Was so light headed and fucked up after heavy deadlifts I pissed all over someone's leg at the urinal.
 
Was doing 100m reps at the track and my coach was starting us. Was in the blocks and in "set" position spewed all over the grass in front of me. Still finished my run though, coach thought it was pretty funny :p

I'm the same. Pushing beyond your limit is better than making excuses with your training buddy. I've been pinned under the bench press and squat bar a few times, but it certainly helped me get better at both.
 
Done the unloading weights and it gets unbalanced and crashes everywhere. Don't the Bench Press fail and almost suffocate. Done the shorts ripping mid set of squats.

When I was younger I had a home gym in the garage. I used to do a set of squats at anytime of the day, just because I liked squatting. One morning before work I thought I'd do a set, no warm up. Thought, I've never done 200kg or close. Load the bar up, walk it out, start the decent, woke up on the garage floor.
 
Maybe at basic training when we had to do pullups, stop at 7 but bend one knee so you can do assisted if you can't do 7 on your own. I basically grabbed onto the bar and bent my knee.

I'd joined the gym to get fit for army but like most people did it all wrong and wasted my time.
